With well-developed communication skills you liaise comfortably with a range of internal and external stakeholders. You are an apt problem solver, capable of troubleshooting on your feet. To be suitable for this position, your qualifications, skills and experience will consist of:

- A Registered Nurse Division 1 with current registration with AHPRA, with minimum of 3 years postgraduate experience;

Indigo is a state-wide service providing assessment, service plan development and care plan coordination for clients with complex needs. Predominately the clients are determined eligible for the Multiple & Complex Needs Initiative (MACNI) however Indigo is increasingly working with clients involved with the Transport Accident Commission (TAC) and the National Disability Insurance Scheme (NDIS).

The Practice Lead will provide support and mentorship to staff within the Indigo Program, delivering individual practice supervision whilst working towards staff capacity building and competencies development. The position plays a key role in ensuring the delivery of high quality assessments and reports for the DHHS Divisional Complex Client Panels as well as for TAC and NDIS. In addition you will maintain a small caseload of clients to whom you will provide quality and inclusive care coordination within multi-disciplinary and multi- agency care teams.

The position responsibilities will include:
- Actively working collaboratively with the Program Manager to organise, facilitate, and participate in team processes including; team meetings, case review, reflective practice and team planning;
- Participate in service and organisation planning and design, including development, implementation and review of policies, procedures and protocols;
- Supporting staff with the Clients Service Plan process lifecycle;
- Provide direct practical assistance and implement strategies (e.g. training) to build services' capacity to work with complex clients; and
- Develop and maintain excellent working relationships key stakeholders.

To be considered for this position you will:
- Hold one of the relevant tertiary qualification;
- Possess extensive experience in the mental health, general health, housing, disability, drug & alcohol, justice, youth or child protection systems;
- Have extensive experience working with clients with complex needs and an understanding of the challenges faced by service providers;
- Have demonstrated professional insight and self-awareness, including the ability to learn and grow from constructive feedback and actively work towards improving areas identified; and
