- Formulating individual service plans in partnership with others for participants with appropriate follow-up care including review and referral;
- Ensuring the service remains connected to community health values and services a provided in an integrated and coordinated manner and
- Building strong partnerships with key stake holders including Corrections staff, Area Mental Health staff and other key providers to ensure appropriate information share to meet the needs of participants and the program.

This position would be ideally suited to someone who:

- Is a registered Nurse Division 1 with current practicing license with AHPRA (Victorian Nurses' Board) with minimum of 5 years postgraduate experience;
- Possesses recent demonstrated experience in a mental health, alcohol and other drugs, and / or forensic health setting;
- Has extensive clinical capabilities, including mental health triage and assessment experience for people with complex needs;
- Has an understanding of relevant legislation and regulations related to the delivery of forensic mental health services
- Possesses a Well-developed interpersonal and written and verbal communication skills, and a proven history of being able to illustrate strong EI qualities; and
- Has experience working with people from culturally and socio economically diverse backgrounds. The ability to speak a relevant community language would be an advantage.

With well-developed communication skills you liaise comfortably with a range of internal and external stakeholders. You are an apt problem solver, capable of troubleshooting on your feet. To be suitable for this position, your qualifications, skills and experience will consist of:

- A Registered Nurse Division 1 with current registration with AHPRA, with minimum of 3 years postgraduate experience;
- Experience and knowledge of mental health programs including an understanding of funding models to maximize opportunities for clients, with experience in meeting monthly targets and KPIs;
- Demonstrated professional approach to service delivery and maintenance of key internal and external stakeholder relationships; and
