Utilising your nursing skill set to work closely with the team, whilst contributing to excellent care of clients with severe and complex mental illness in the Western regions of Melbourne. Within this role you are required to develop and maintain collaboration with the North Western Melbourne Primary Health Network which coordinates the Mental health Nurse Program, including the Intensive Support Service and Targeted Psychological Services.

As our Mental Health Nurse, your responsibilities will include:

- Providing coordinated services to clients who meet the MHN Program eligibility and live within the Western Melbourne Primary Health Network areas;
- Providing client treatment and review in accordance within MHN goal directed care plans and GP mental health plans; and
- Maintaining professional, accurate and contemporaneous client file notes, assessments, care plans and all other documentation pertaining to client's management.

With well-developed communication skills you liaise comfortably with a range of internal and external stakeholders. You are an apt problem solver, capable of troubleshooting on your feet. To be suitable for this position, your qualifications, skills and experience will consist of:

- A Registered Nurse Division 1 with current registration with AHPRA, with minimum of 3 years postgraduate experience;
- Experience and knowledge of mental health programs including an understanding of funding models to maximize opportunities for clients, with experience in meeting monthly targets and KPIs;
- Demonstrated professional approach to service delivery and maintenance of key internal and external stakeholder relationships; and

We're seeking a qualified nurse who has a passion for working in this challenging, yet extremely rewarding field of mental health nursing. Utilising your nursing skill set to work closely with the team, whilst contributing to excellent care of clients with severe and complex mental illness in the Western regions of Melbourne. Within this role you are required to develop and maintain collaboration with the North Western Melbourne Primary Health Network which coordinates the Mental health Nurse Program, including the Intensive Support Service and Targeted Psychological Services.

As our Mental Health Nurse, your responsibilities will include:

- Providing coordinated services to clients who meet the MHN Program eligibility and live within the Western Melbourne Primary Health Network areas;
- Providing client treatment and review in accordance within MHN goal directed care plans and GP mental health plans;
- Maintaining professional, accurate and contemporaneous client file notes, assessments, care plans and all other documentation pertaining to client's management;
- Ensuring standards and legislative requirements are met and maintained whilst managing the clinical environment in the areas of:
- Infection Control
- Accreditation
- Occupational Health and Safety
- Promoting and representing cohealth as a caring, professional and client-focused organisation and its range of primary health and social support services.

With well-developed communication skills you liaise comfortably with a range of internal and external stakeholders. You are an apt problem solver, capable of troubleshooting on your feet. To be suitable for this position, your qualifications, skills and experience will consist of:

- A Registered Nurse Division 1 with current registration with AHPRA, with minimum of 3 years postgraduate experience;
- Experience and knowledge of mental health programs including an understanding of funding models to maximize opportunities for clients, with experience in meeting monthly targets and KPIs;
- Demonstrated professional approach to service delivery and maintenance of key internal and external stakeholder relationships;
- Possesses the aptitude to work under pressure, remain calm in challenging situations and ability to work with clients who may present with mental health concerns; and
- A curious and positive approach to your work life, continuously striving to contribute to the growth and development of cohealth as a provider of choice.

These roles will mainly be focused around food production and plastic manufacturing. These roles will be ongoing and will be starting immediately. You must be able to be on call for any role offered.
