About the Role 
This position has a focus on developing the Aged Care program for renal dialysis clients living in Alice Springs. The position has an operational emphasis, primarily around client service coordination, case management and the provision of administration support for the aged care programs in Alice Springs and Mt Liebig.
The role will include operational, client coordination and case management duties as follows:
Operations:
Maintain appropriate records to ensure the organisation is meeting its contractual and regulatory requirements. Maintain Client Information including the maintenance of any Client Information Management System (CIMS) and provide support to other users. Provide clerical and administrative coordination and support to the Business Manager for the Aged Care program, including: Coordinating and ensuring compliance with reporting schedule's as required under funding agreements; Preparing draft reports for funding bodies for approval, and submission by the Manager/ CEO; Monitoring program compliance with contractual requirements.
Liaise with and support the Aged Care Coordinator at Mount Liebig on operational issues, keeping the Business Manager informed of any emerging developments and progress. Provide general customer/client service and support, including acceptance and registration of aged care complaints*
(*Following the WDNWPT complaints handling policy and procedures) Provide support to management in organising quotations, purchase orders, and coordinate delivery of goods and services for aged care as per the organisation purchasing policy. Provide administrative support for Aged Care meetings.
Client Coordination and Case Management:
Develop written care plans which includes identifying key goals with the client and according to referral and assessment needs; Follow intake assessment procedures relevant for client type; Conduct client and physical environment risk assessments and assist to identify risk mitigation strategies; Negotiate care supports and services with the client and carers; Complete required reporting e.g. DEX, Medicare claims, home care package budgets & statements; Identify physical aids and equipment required by clients and liaise with the relevant suppliers, as required; Provide case management, supporting the organisation, carers and families to meet the identified needs of the clients; Regularly monitor care supports against stated goals and regular reassessment; Ensure client progress notes are appropriately maintained and ensure timely reporting and recording of client events; Refer clients to external support agencies where there is an identified need; Ensure effective communication regarding client health & wellbeing, including involvement and contribution to regular and consistent team meetings and handovers; Advocate for clients and their carers as required.
