cohealth Victims Assistance Program (VAP) provides services to victims of crime against the person and related victims of homicide, to assist them effectively manage and recover from the effects of the crime. This program is primarily based in Footscray but will also be co-located with police in three areas and an intake hub in Maribyrnong.

The primary responsibilities of the Victims Support Worker (Case Manager) is to;

- Provide trauma informed case management to victims of violent crime, particularly those of a serious and/or complex nature (note that VAP is not a crisis intervention service);
- Provide case management (including outreach) to ‘related victims' of homicide;
- Provide psycho-education to victims, police and the community;
- Support victims of crime to effectively navigate the criminal justice system;
- Work collaboratively with the police and other service delivery agencies;
- Provide ethical, professional and responsible goal directed, trauma informed and client-centred case management to victims of crime - particularly those of a serious and/or complex nature as well as case management to ‘related victims' of homicide; and
- Where the Program requires, provide initial intake phone calls.

About You
To be considered for this exciting opportunity you will possess:

- Tertiary qualifications in Social Work with eligibility to membership of the AASW or an equivalent qualification;
- Ability to work sensitively with people from diverse cultures, ages, sexuality, ability and gender;

- A high level of experience in case management of diverse populations and experience/understanding of trauma informed practice;

Our Older Persons High Rise (OPHR) Support program is based in high rise accommodation, within the Public Housing estates of the Inner North and West, which are allocated to older people. The OPHR program ensures that isolated and vulnerable residents have access to support and services. The program aims at increasing residents' sense of safety, independence and security while enhancing their social and community involvement. We provide on-site presence, low level monitoring of the whole high rise population, one to one support work and on site health and social activities.

We operate using a team approach and a rotation system to support workers. We provide residents with a sense of variety and exposure to different sets of skills. There is an expectation that you will be rotated through at least two public housing estates and have capacity to assist, where possible, at any of the cohealth OPHR programs.

Your key duties and responsibilities will include:

- Working within a team based model of service delivery, to locate and assertively engage marginalised clients;
- Monitoring the health and wellbeing of vulnerable clients, especially those who do not have case managers or support workers;
- Developing goal directed care plans for clients of the program;
- Developing new activities, where possible in partnership with other agencies; and
- Maintaining client files including keeping case notes, statistical/program records according to organisational policy and funding requirements;

Key duties and responsibilities will include;
- Acting within the Nursing and Midwifery Board of Australia codes of conduct, professional standards, guidelines, legislation and regulations;
- Performing health assessments to ensure appropriate triage and referral pathways for clients;
- Contributing to the development of Goal Directed Care Plans;
- Developing and executing immunisation plans including, childhood immunisations, Refugee immunisation catch up plans; and
- Providing advocacy support as required to reduce barriers of access particularly for cohealth target groups.

About You
To be considered for this exciting opportunity you will possess the following:
- Registered Nurse Division 1 with current practicing registration with AHPRA; with a Minimum 3 years Post Graduate experience;
- Current qualification as Nurse Immunisation Provider;
- Is proficient with ICT systems, plus has the ability to pick up new systems quickly;
- Possesses the aptitude to work under pressure, remain calm in challenging situations and ability to work with clients who may present in an intoxicated state or with mental health concerns; and
- Has the capability to multi-task and prioritise effectively to achieve quality outcomes in a busy, client-focussed environment.
