Our Older Persons High Rise (OPHR) Support program is based in high rise accommodation, within the Public Housing estates of the Inner North and West, which are allocated to older people. The OPHR program ensures that isolated and vulnerable residents have access to support and services. The program aims at increasing residents' sense of safety, independence and security while enhancing their social and community involvement. We provide on-site presence, low level monitoring of the whole high rise population, one to one support work and on site health and social activities.

We operate using a team approach and a rotation system to support workers. We provide residents with a sense of variety and exposure to different sets of skills. There is an expectation that you will be rotated through at least two public housing estates and have capacity to assist, where possible, at any of the cohealth OPHR programs.

Your key duties and responsibilities will include:

- Working within a team based model of service delivery, to locate and assertively engage marginalised clients;
- Monitoring the health and wellbeing of vulnerable clients, especially those who do not have case managers or support workers;
- Developing goal directed care plans for clients of the program;
- Developing new activities, where possible in partnership with other agencies; and
- Maintaining client files including keeping case notes, statistical/program records according to organisational policy and funding requirements;

To be successful for this role you will have;

- Experience working in the health/community sector including advanced skills in community development, care facilitation and assertive outreach;
- The confidence working in a satellite campus on a public housing estate;
- Demonstrated competency in delivery of group programs;

The Program
HOMHS is a multi-disciplinary, multi-agency team comprising staff from cohealth, Inner West Area Mental Health Service and Launch Housing, alongside a strong partnership with McAuley Community Services for Women. HOMHS provides case management support to people who experience severe and enduring mental illness and a history of repeated homelessness. HOMHS provides the equivalent Area Mental Health Service clinical case management service to all consumers of the program.

The role of Community Mental Health Worker is to provide a service that aims to enhance quality of life, build life skills and ensure access to both community mental health and broader health and community services. Focusing on client service delivery through facilitative and direct care engagement using CRM, underpinned by the principles of trauma informed care; family centred practice and culturally appropriate practice. We employ a flexible, individualised approach, working in consultation with clients to assess needs and develop a range of client directed goals. Care plans and activities offered promote choice, acknowledge individual strengths and facilitate inclusion within the community.

Your daily duties will be diverse however, in essence will include:
- Providing case management support and advocacy to consumers with a range of complex needs who are accessing HOMHS, addressing specific needs;
- Developing and implementing client-directed, strengths-based individual client service packages from entry to exit;
- Working collaboratively with clients and the HOMHS team to conduct comprehensive assessments, identifying needs, aspirations and goals of clients;
- Developing and maintaining working relationships with key local and internal stakeholders;
- Facilitating client access to community resources, peer leadership initiatives and services including the Volunteer - Coordination Program and brokerage; and
- Providing support to prepare clients who are transitioning to the NDIS.

- A tertiary qualification in a relevant sector and/or commensurate experience in a related area, as determined by cohealth;

Our Clinical Nurse Consultant role forms an integral part of the North Western Metropolitan Integrated Pharmacotherapy Network (NWMIPN) Addiction Medicine Specialist service. The role exists to coordinate, support and advise an Addiction Medicine Specialist and Medication-Assisted Treatment of Opioid Dependence (MATOD).

Based on a chronic disease management (CDM) approach for addiction, and within a framework of primary care, harm reduction and recovery. The NWMIPN is based on a partnership with representation across the MATOD service delivery spectrum including cohealth, Western Health Drug Health Services, general practice through Primary Health Networks and consumer voices through Harm Reduction Victoria.

Within this role your responsibilities will comprise of;

- Supporting the establishment of an out-posted model of care, involving linking MATOD prescribers with an Addiction Medicine Specialist (AMS) to provide secondary consultation for complex clients;
- Providing specialist AOD nursing support role for AMS services, including managing the AMS' calendar bookings and commitments within the area, and ensure that all funded time allocation is fully utilised;

As a Community Mental Health Worker you will work closely with the team to provide flexible, intensive recovery-oriented support to participants. Collaborate with the client, client's care team and support systems including families and significant others, to help manage and decrease psychosocial stressors. Utilising the Collaborative Recovery Model, negotiating with residents to determine the appropriate level of support necessary over time, and work towards agreed goals during their stay.

This positions will ideally be suited to an individual who possess:
- Demonstrated experience in community based support of people with mental illness and/or psychiatric disability or other equivalent/relevant;
- Demonstrated ability to work collaboratively with the client's treatment team and support systems including families and significant others;
- Culturally and linguistically diverse focus - this is sensitivity to, and understanding of, the needs of clients from culturally and linguistically diverse backgrounds as well as people of different gender and ages;
